To get started and make an appointment, NEW clients MUST e-mail
Teen Talk Line at JoelleMFT@TeenTalkLine.org to briefly describe the presenting problem. If you are under 18 years of age, Teen Talk Line must also receive an e-mail from your caregiver consenting to counseling by Joelle Jacobson, M.A., L.M.F.T, P.P.S., Neurofeedback Clinician, Treatment Placement Specialist. In your caregiver’s e-mail, please include any information regarding custody agreements and consent to use the credit card for counseling.
After you have received an e-mail response from Joelle, you must fill out and sign the appropriate intake form, TeleHealth consent form and disclaimer form before counseling can begin. You need to fax it back with your signature. If you are a teen, you need to fax it back with your signature and your caregiver’s signature to 855-833-6411.
If you are a teen and your parents are divorced, your parents must have joint-legal custody for one parent to sign off on treatment. If your parent has sole custody, that parent must sign the TeleHealth consent form. If the custody agreement specifies that both parents must sign off on mental health treatment, both parents must sign the TeleHealth form. If accessible, please fax a copy of the custody agreement for Joelle Jacobson’s records.
You may not set up a first appointment on the scheduling calendar or pay for the time allotted, until you receive an email response from Joelle confirming the receipt and completion of your forms.
Once you receive a response, proceed to the Teen Talk Line schedule to sign up for an appointment. You will only be able to see your Login name and the available time slots. The filled time slots and names of other people utilizing Teen Talk Line will be shaded for the purpose of confidentiality. If you experience any difficulty scheduling an appointment, please e-mail Joelle directly.
After you have scheduled your appointment, you will need to pay for the session through the online credit card portal. At the time of your appointment, Joelle will call you on the preferred phone number you specified on your intake form. If you would like to be called on a different phone number then what is listed on your form, please send Joelle an e-mail before your appointment time.
